Family Physicians at Summer Grove, PC

Before your first visit, please call our office at 770.252.3783. 
You will be given a User Name and Password for the Patient Portal and some basic information will be collected. 
Please then return to fpsg.org and click on the Patient Portal tab.  Follow the instructions below to complete your New Patient information.
At every visit, bring a picture ID, your insurance card, all medications, and your copay or deductible.

Welcome to Family Physicians at Summer Grove!

We look forward to taking care of you and your family.

Before your first visit, please enter the Patient Portal.
Use the User Name and Password you chose when you called the office.

The Portal is very easy to navigate, but here are detailed instructions if you need help.

Personal Information
Look on the left, under Manage.
Click on Personal Information.   Review and correct if needed, the information.
Complete any blank boxes.
Click on "To Update click here"
Do the same for Additional Information.

Health History Form
Next, complete the Health History Form
Click on Medical History. 
Please click on any medical problems you have.
At the bottom of the page, click on Submit and Next.
The next page is your Family History.
Please click on any medical problems your family members have had.
At the bottom of the page, click on Submit and Next.
The next page is Social History.
Please answer these questions and at the bottom of the page, click on Submit and Next.
The next page is the Review of Systems.
Please answer ALL of the questions, either Yes or No.
Only answer Yes if you have had these symptoms in the past year and if they have been severe enough to either go the the doctor or consider going to the doctor.
At the bottom of the page, click Submit.
It will then take you to a page that says your questionnaire response has been submitted sucessfully.
Click Next.

Surgical and Allergies Information

Please type in any surgeries or operations you have had and the month and year of the surgery.

Here are the spellings of some common surgeries:

Tubal Ligation
Hysterectomy
Tonsillectomy
Adenoidectomy
Masectomy
Appendectomy
Cholecystectomy (gallbladder removal)
D&C (Dilation and Curettage)
C-Section (Cesarean section)

Next, type in the name and month and year of any hospitalizations, except for childbirth.

Here are the spellings of some common hospitalizations:

Pneumonia
Dehydration

Next, enter any drug allergies and any severe allergies to foods, etc.  You do not need to include allergies to pollen/hay fever.
Please enter the type of reaction you had.

Here are the spelling of some common medication allergies:

Penicillin
Erythromycin
Sulfa
Codeine

At the bottom of the page, click Submit.
